Job summary

Lehigh Valley Health Network is seeking a Registered Nurse to join the Hospice Pocono team as a Case Manager. The role manages home-based patient care, collaborates with an interdisciplinary team, and ensures care plans meet regulatory standards.

The position is full-time, daytime hours in East Stroudsburg with a $10,000 sign-on bonus. Must have an active RN license in PA and ability to travel to patient care sites.

Qualifications

• Specialized nursing program with NLN accreditation.
• 1 year recent direct care experience in healthcare.
• Knowledge of patient education techniques and principles.
• Ability to travel to patient care assignments.
• Excellent time management and organizational skills.
• Knowledge of Conditions of Participation and regulatory data collection.
• Strong verbal and nonverbal communication skills.

Responsibilities

• Coordinating and directing care for home care/hospice patients with interdisciplinary team.
• Developing and revising care plans aligned with physician orders and participation requirements.
• Providing patient and caregiver education to achieve goals and outcomes.

Education

Specialized Diploma NLN approved/accredited nursing program

Associate’s Degree in Nursing

Bachelor’s Degree in Nursing
