Sage GroupHealthCare Staffing is seeking a Hospice Registered Nurse Case Manager to cover home health in Tullahoma, TN. This full-time role may include travel/contract considerations with a 40-hour week.

Responsibilities include care coordination, Start of Care and admissions, comprehensive case management, and patient-family education to enable home-based care. Territory includes Manchester and Woodbury area.

Qualificações

• RN with 2 years of experience required.
• Hospice/Home Health experience within last 3 years required.
• HCHB charting experience required.
• Valid driver’s license and auto insurance required.
• COVID vaccination or exemption required.

Responsabilidades

• Provide and coordinate care for hospice patients at home.
• Develop and implement nursing care plans for Start of Care and admissions.
• Provide case management for assigned patient load.
• Support pain management to help patients stay at home.
• Educate patients and families on disease management.
• Case manage and oversee interdisciplinary teams.
• Chart and document patient visits within 24 hours.
• Territories covered: Manchester / Woodbury – Coffee and Cannon counties.
