Job summary

St. Croix Hospice in Stevens Point, WI is seeking a Registered Nurse Case Manager to provide 1:1, whole-person care to patients and families in their homes.

You will coordinate care plans with an integrated team, support families through complex decisions, and deliver compassionate, respectful care. The role requires RN licensure and at least 1 year of acute care experience; hospice/home health is preferred.

Qualifications

• Associates, Bachelors, or Masters of an accredited nursing program (RN).
• 1+ years of recent acute care experience required; hospice/home health experience preferred.
• Develop individualized care plans with the patient and family, updating as needs evolve.

Responsibilities

• Provide 1:1, whole-person home-based care to patients and families.
• Coordinate individualized care plans with an integrated care team.
• Educate patients and caregivers on care techniques and medications.
• Rotate on-call duties and ensure seamless care transitions.
