St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver 1:1, whole-person care to patients and families in home settings.

You will coordinate individualized care plans with an integrated care team, support families through complex decisions, and provide compassionate, professional care with autonomy in practice. Requirements include RN licensure (Associates, Bachelors, or Masters from an accredited nursing program), 1+ year acute care experience (hospice/home health preferred),

Qualifikationen

• 1+ years of recent acute care experience required; hospice/home health experience preferred
• Provide 1:1, whole-person care to patients and families
• Coordinate individualized care plans with integrated care team
• Educate patients and caregivers on care techniques, medications, diet, and symptom management
• Rotate on-call duties and ensure seamless care transitions
• Must have reliable transportation, valid driver’s license, and proof of auto insurance
