Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver individualized, holistic end-of-life care in patients' homes in Wisconsin. You will lead care planning, assessments, and symptom management with empathy and clinical excellence.

Requirements include an RN license with 1+ year acute care experience; hospice/home health experience preferred. On-call duties and reliable transportation are part of the role.

Qualifications

• RN licensed and eligible to practice in Wisconsin.
• 1+ year of acute care experience; hospice/home health preferred.
• Conduct initial and ongoing assessments to determine hospice needs.
• Develop individualized care plans with patient and family; update as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet and symptom management.

Responsibilities

• Provide holistic in-home nursing care to hospice patients.
• Lead meaningful end-of-life conversations with patients and families.
• Collaborate with care team to ensure symptom management and comfort.
• Coordinate transitions and on-call coverage for continuous care.
