Job summary

St. Croix Hospice in Mount Pleasant, WI is seeking a Registered Nurse Case Manager to deliver 1:1 in-home hospice support.

You will partner with an interdisciplinary team to develop individualized care plans, guide end-of-life conversations, and ensure comfort with professionalism and empathy. The role includes on-call duties, comprehensive assessments, patient and family education, and coordination of care transitions.

Qualifications

• Graduate from an accredited nursing program and RN licensed.
• 1+ years of acute care experience required; hospice/home health preferred.
• Ability to conduct comprehensive assessments and develop care plans.

Responsibilities

• Provide 1:1 in-home hospice care and support.
• Coordinate with interdisciplinary team to update care plans.
• Guide end-of-life conversations with patients and families.
• Rotate on-call duties and ensure smooth care transitions.
