Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide 1:1, whole-person home care for patients and families, coordinating with an integrated care team to build and adjust care plans, and supporting families through complex decisions with empathy and professionalism.

The role requires an RN with 1+ years of acute care experience (hospice/home health preferred). Travel to patient homes is part of the job, with a schedule from 8:00am to 5:00pm and on-call responsibilities.

Qualifications

• Current RN license in Wisconsin
• 1+ years of acute care experience required; hospice/home health preferred
• Ability to develop individualized care plans and educate patients and caregivers

Responsibilities

• Conduct initial and ongoing comprehensive home assessments to determine hospice needs
• Develop and update individualized care plans with patients and families
• Administer medications and treatments as prescribed in patient homes
• Coordinate with care team to ensure smooth transitions and on-call coverage
