Job summary

Resource 1 Staffing is seeking a Hospice Registered Nurse (RN) Case Manager in Fremont, NE for a full-time schedule. The role focuses on coordinating home-based hospice services, patient and family education, and interdisciplinary collaboration to deliver compassionate end-of-life care.

The RN will assess needs, develop and adjust care plans, administer prescribed treatments, and support families in understanding disease progression and care options.

Qualifications

• Graduation from an accredited school of nursing.
• The ability to work efficiently with little direct supervision.
• A valid driver's license and an insured automobile that is in good working order.
• 1-2 years of recent acute care experience in an institutional setting (required). 1-2 years of recent home care experience (preferred).
• Understands hospice philosophy, comfortable with death/dying and providing specialized care to the terminally ill.
• Displays empathy for the needs of the ill, injured, frail and impaired.
• Provides a calm demeanor when in a patient’s home.

Responsibilities

• Completes an initial, comprehensive assessment of patient and family to determine hospice needs and conducts a complete physical assessment.
• Provides professional nursing care using the nursing process and develops a care plan with goals based on nursing diagnosis.
• Initiates and revises the plan of care as patient needs change, and coordinates with the physician.
• Counsels patient and family in meeting nursing and related needs; provides health instructions and supports self-care at home.
• Participates in on-call duties and arranges equipment and services; supervises ancillary staff as needed.
• Maintains accurate clinical notes, communicates with physicians and the interdisciplinary team, and ensures continuity of care.
