Qualifications

• Graduation from an accredited school of nursing.
• 1-2 years of acute care experience; 1-2 years home care preferred.
• Valid driver’s license and insured automobile in good working order.
• Hospice philosophy understanding and comfort with end-of-life care.

Responsibilities

• Performs comprehensive patient and family assessments and updates care plans accordingly.
• Provides professional nursing care following the nursing process.
• Initiates and revises the plan of care as patient needs change.
• Records patient status and pain/symptom management changes.
• Coordinates care with physician and interdisciplinary team.
