Generations Hospice is hiring a Registered Nurse Case Manager to deliver high-quality hospice care in Wisconsin. The role involves coordinating patient care with an interdisciplinary team, developing individualized care plans, and administering treatments to relieve symptoms with compassionate support for families.

This full-time position includes a $5,000 sign-on bonus, a Monday–Friday schedule, and opportunities for professional growth within a growing organization that values collaboration

Qualifications

• Requires Wisconsin RN license and graduate of accredited nursing program.
• One year nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care.

Responsibilities

• Assess/ monitor physical, emotional, and psychological needs of patients
• Create hospice care plans that align with the patient's wishes and goals
• Direct nursing care: administering medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support the patient's family and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch
