Job summary

Trustbridge Hospice, part of Empath Health, is seeking an Admissions Nurse to join the Admissions Team serving Broward County. The role includes evaluating referred patients, educating families, coordinating care, and liaising with hospitals and physicians. Schedule: Friday through Sunday, 9am–9:30pm.

Qualifications include Florida RN licensure, BSN and hospice/BSN preferred, BLS, and basic computer/EMR skills. Transportation and license required.

Qualifications

• Registered Nurse currently licensed in the State of Florida.
• BSN preferred and hospice certification preferred.
• Current Basic Life Support Certification.

Responsibilities

• Evaluate patients referred for hospice services as the primary nurse.
• Educate patients and families on hospice care and continuum of care.
• Coordinate individualized hospice plans of care with physicians and liaisons.
• Act as liaison between Trustbridge Hospice and hospitals, physicians, facilities.

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and motivated Registered Nurse (RN) to join our Admissions Team servingBroward County. This is a meaningful opportunity for an RN who is passionate about guiding patients and families through one of life’s most significant transitions with empathy, clarity, and dignity. You will work the following schedule: Friday/Saturday/Sunday 9am - 9:30pm

What you’ll Do

As an Admissions Nurse , you will play a vital role as both a clinical expert and a trusted guide for patients, families, and healthcare partners:

• Serve as the primary nurse responsible for evaluating patients referred for hospice services
• Educate patients and families on hospice care and clearly communicate all components of our continuum of care
• Collaborate with referring physicians and liaisons to coordinate individualized, hospice‑appropriate plans of care
• Act as a key liaison between Trustbridge Hospice and hospitals, physicians, long‑term care facilities, and other healthcare professionals
• Promote and advocate for the hospice philosophy through education and relationship‑building within the healthcare community
• Your work ensures patients receive timely, compassionate care while helping families feel informed, supported, and confident in their hospice journey.

What You’ll Need

• Registered Nurse currently licensed in the State of Florida.
• BSN preferred
• Hospice certification preferred.
• Current Basic Life Support Certification.
• Possess the ability to assess physical and psychosocial needs of patient and family.
• Minimum of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
• Understanding of and ability to apply characteristics of Hospice Nursing, case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
• Basic computer skills and EMR software experience a plus.
• Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
