Job summary

Serenity Hospice Care Pennsylvania is seeking a compassionate Registered Nurse (RN) Hospice Case Manager to join our Monroe County team. This role focuses on delivering high-quality, patient-centered end-of-life care in patients’ homes and other settings, coordinating with an interdisciplinary team to ensure comfort and dignity.

The RN will conduct comprehensive assessments, develop individualized care plans, and educate families on disease progression and symptom management.

Qualifications

• Current Pennsylvania RN license is required.
• Minimum 1 year of nursing experience (hospice/home health or case management preferred).
• Strong clinical, organizational, and communication skills are essential.
• Compassionate, patient-focused approach to care.
• Valid driver’s license with reliable transportation.

Responsibilities

• Manage and coordinate care for an assigned hospice caseload.
• Perform comprehensive patient assessments and develop individualized plans of care.
• Provide skilled nursing visits in patients’ homes and other care settings.
• Collaborate with physicians, social workers, chaplains, aides, and other team members.
• Educate patients and families on disease progression, symptom management, and medications.
• Monitor patient status and adjust care plans as needed.
• Ensure accurate, timely clinical documentation in accordance with hospice regulations.
• Participate in interdisciplinary team meetings and care planning.
• Serve as a patient and family advocate throughout the hospice journey.
