Job summary

Central Coast VNA & Hospice (VNA) in Salinas, CA is seeking a Hospice RN Case Manager for a full-time role. The position focuses on delivering skilled nursing care and coordinating home health and hospice services to patients and families, prioritizing comfort and quality of life.

The Hospice Case Manager will collaborate with physicians and the interdisciplinary team, educate families, and facilitate transitions between care levels as needed.

Qualifications

• Active Registered Nurse license in California.
• Minimum of one year of Home Health or Hospice experience.
• Knowledge of home health, hospice, and palliative care.
• Current CPR certification.
• Ability to collaborate with physicians and interdisciplinary team.

Responsibilities

• Collaborate with physicians and the interdisciplinary care team to coordinate home care for patients, families and caregivers.
• Provide skilled nursing care and supportive services focusing on patient comfort, safety, and quality of life.
• Refer and coordinate other disciplines and facilitate transitions to other levels of care.
• Educate patients and families and prepare for discharge when appropriate.

VNA is looking for a RN Case Manager.

The Hospice Case Manager collaborates with physicians and the interdisciplinary care team to identify, coordinate, and address the home care needs of patients, families, and caregivers. The Hospice Case Manager provides skilled nursing care and supportive services using the nursing process, with a focus on patient comfort, safety, and qualify of life.

They refers and coordinates other disciplines and specialty services and facilitates transitions to other levels of care or services, as needed. Manages patient care in accordance with agency standards, applicable regulations, and payer requirements. Provides education and support to patients, families, and caregivers and prepares patients and families for discharge when appropriate.

Education/Experience/Skills/Qualifications

• Active Registered Nurse license in the California
• Minimum of one year nursing Home Health or Hospice experience required
• Knowledge of home health, hospice, palliative care, and other agency services preferred
• Current CPR certification
• Ability to work collaboratively with physicians, families, caregivers, and the interdisciplinary team members
