Job summary

Central Coast VNA & Hospice (VNA) is seeking an RN Case Manager in Monterey, CA to deliver skilled home health care in collaboration with physicians and an interdisciplinary team. This full-time role focuses on patient-centered care, safety, and quality of life, with planning, coordination, and discharge planning as part of daily duties.

The position requires California RN licensure, at least one year of nursing experience, home health experience, and willingness to travel across the Monterey

• Minimum of one year nursing experience.
• Home Health experience required.
• Two years of recent medical-surgical nursing experience preferred.

Responsibilities

• Assess, plan, coordinate and deliver skilled home health care with the care team.
• Coordinate services with internal and external providers and ensure payer guidelines compliance.
• Prepare patients and caregivers for discharge when appropriate.
• Connect patients with community resources to support continuity of care.

Salary Range: $60.92 To $66.08 Hourly

Job Summary: The RN Case Manager works collaboratively with physicians and the interdisciplinary care team to assess, plan, coordinate, and deliver skilled home health care to patients and their families. This role focuses on providing supportive, patient-centered care that promotes comfort, safety, and quality of life. The nurse utilizes the nursing process to manage patient care, coordinates services with internal and external providers, and ensures compliance with payer guidelines. The RN Case Manager also prepares patients and caregivers for discharge when appropriate and connects them with community resources to support continuity of care.

Education/Experience/Skills/Qualifications:

• Active Registered Nurse license in the California
• Minimum of one year nursing experience required
• Home Health experience required
• Two years of recent medical-surgical nursing experience preferred
• This position is based in Monterey, CA and requires daily travel throughout the Monterey Peninsula and surrounding coastal communities. Candidates must reside within a reasonable commuting distance that allows efficient coverage of the assigned territory.
