Job summary

Interim HealthCare in Michigan is seeking an experienced Hospice RN Case Manager to deliver compassionate home‑based nursing for patients at end of life. You’ll collaborate with physicians and CNAs, perform assessments, and educate families.

This salaried role offers a $75k-$100k range plus weekly bonus opportunities, comprehensive benefits, ESOP eligibility after 1 year, and flexible assignments to support work‑life balance.

Qualifications

• Active, unrestricted RN license in Michigan.
• Minimum 1 year of clinical experience as an RN, preferably in Hospice.
• Current CPR certification.
• Reliable transportation with a valid driver's license and current car insurance.

Responsibilities

• Provide home-based care to patients with terminal illness.
• Work with a team of physicians and oversee CNAs providing patient care.
• Examine patients, perform assessments, document and report changes to the physician.
• Educate patients on plan of care, prescribed medication, diet and exercise.
• Coach family members on the patient’s plan of care, medication and home safety.
• Ensure goals are met and coordinate discharge from services.
