Job summary

The Pines Home Health and Hospice is seeking a compassionate Hospice RN to serve as a trusted case manager, delivering in-home care across communities in Arizona. You will guide families through care decisions, ensure comfort and dignity, and support patients during life’s most tender moments.

You will complete comprehensive assessments, develop care plans, administer medications, and coordinate with an interdisciplinary team to improve patient outcomes and quality of life.

Qualifications

• Current RN license in the State of Arizona.
• Minimum of 2 years of nursing experience, hospice or home health experience preferred.
• Homecare Homebase (HCHB) experience preferred, not required.
• Valid driver’s license and reliable, insured vehicle.

Responsibilities

• Complete comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans and revise as needed.
• Provide skilled nursing care, administer medications, and manage symptoms.
• Educate patients and families on care techniques, medications, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Collaborate with an interdisciplinary team and participate in quality improvement initiatives.
