Job summary

The Pines Home Health and Hospice in Arizona is seeking a dedicated Hospice CNA to join our home-based care team. Under the supervision of an RN, you will provide compassionate personal care, monitor patient conditions, and help families manage daily needs in the comfort of patients' homes.

You will build meaningful relationships with patients and families, educate caregivers, and collaborate with an interdisciplinary team to deliver high-quality care.

Qualifications

• Current Arizona CNA license.
• Valid driver’s license and reliable vehicle.
• Strong clinical skills with patient-centered focus.
• Ability to work independently and collaborate with an RN.

Responsibilities

• Build meaningful relationships with patients and families.
• Assist with personal care needs (bathing, dressing, mobility).
• Educate and empower patients and caregivers to manage daily needs.
