Comfort Hospice in Pahrump, NV is seeking a dedicated Hospice RN Case Manager to join our team. You will perform comprehensive home-based assessments, develop tailored care plans, and guide families through care decisions with compassion and expertise.

Travel throughout the Pahrump area is required. The role emphasizes patient-centered nursing, clinical excellence, and collaboration with an interdisciplinary team to deliver high-quality hospice care.

Qualifications

• Current RN license in Nevada.
• Minimum of 2 years of nursing experience; hospice or home health preferred.
• Strong clinical skills and patient-centered care.
• Ability to work independently.

Responsabilités

• Perform comprehensive assessments of patients and families to determine hospice needs.
• Develop and implement individualized care plans; revise as needed.
• Provide skilled nursing care and administer medications; manage symptoms.
• Educate patients and families on care techniques, medications, and comfort measures.
• Maintain accurate clinical documentation and communicate changes to physicians.
• Participate in interdisciplinary team meetings and quality improvement initiatives.
• Provide emotional support and counseling to patients and families.
• Supervise hospice aides and delegate responsibilities.
• Collaborate with an interdisciplinary team to maintain high-quality care.
• Participate in holiday on-call rotation as needed.
