Job summary

CompassionCare Hospice in Las Vegas, NV is seeking a dedicated RN Case Manager to provide expert, patient-centered hospice nursing care. The role emphasizes compassionate, independent work with team support in a Home/Hospice setting.

Responsibilities include assessing patients, developing care plans, administering treatments, managing pain and symptoms, and guiding families. Eligible candidates hold NV RN licensure with hospice experience preferred and driving eligibility.

Qualifications

• Graduate of an accredited nursing program with current NV licensure
• One year nursing experience required, 3+ preferred
• Experience in hospice or similar setting preferred
• Knowledge of hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with insured vehicle

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients
• Create hospice care plans aligning with patient wishes and goals
• Direct nursing care including medications, treatments, and interventions
• Provide pain and symptom management
• Educate and support patients’ families and caregivers
• Collaborate with interdisciplinary team
• Maintain accurate and timely documentation
• Participate in on-call rotation as required by local branch

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required , 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
• Possess and maintain current CPR certification in states where required.
