Hospice RN Case Manager

Provides professional nursing care to hospice clients as prescribed by the physician, in compliance with regulations and organizational policies. The RN is responsible for client assessment, care planning, therapeutic intervention, and supervision of client care, acting as the case manager for hospice patients. Collaborates with the physician and team to manage symptoms, address grief/loss issues, and implement the hospice plan of care. Provides support to Hospice Aides and LPN/LVNs as needed.

Essential Functions

1. Maintains current knowledge of applicable laws, regulations, organizational policies, and adheres to ethical standards.
2. Informs clients and caregivers about services, responsibilities, and rights, including hospice benefits.
3. Develops and implements individualized, outcome-focused care plans to improve quality of life and dignity.
4. Refers and coordinates care with other disciplines and services.
5. Uses assessment skills and nursing judgment to formulate interventions and make recommendations.
6. Reports changes in patient condition promptly and obtains necessary orders.
7. Re-evaluates and updates care plans based on patient progress.
8. Assesses learning needs and provides education on disease, self-care, and end-of-life issues.
9. Responds to physical, psychological, or spiritual changes in patients.
10. Models teamwork and respects cultural diversity.
11. Maintains knowledge of pain and symptom management, utilizing evidence-based protocols.
12. Prepares patients and families for disease progression and death.
13. Documents care accurately and timely in the electronic system.
14. Ensures timely submission of client data and maintains confidentiality.
15. Participates in team conferences and acts as a case manager, coordinating with other health team members.
16. Acts independently and responsibly, demonstrating professional growth.
17. Supervises and mentors staff, including HPA and LPNs, and participates in orientation and education.
18. Performs additional duties as required.

Minimum Qualifications

• Graduate of an approved Nursing program
• Current RN licensure in Iowa
• At least one year of professional nursing experience
• Preferred: home healthcare/hospice experience
• Strong communication and interpersonal skills, with emphasis on grief/loss management
• Valid driver’s license and reliable transportation
• Commitment to organizational ethics and compliance
