Job Description

This is a full-time, work-from-home position. The candidate must be located in the Cedar Rapids, IA area due to regular local travel (80%) for in-person patient appointments.

Join our compassionate team and help make a positive difference in an injured person’s life. As a Field Case Manager, you will:

  • Work closely with treating physicians/providers, employers, customers, legal representatives, and the injured/disabled person to create and implement a treatment plan that returns the injured/disabled person back to work appropriately, ensure appropriate and cost-effective healthcare services, achievement of maximum medical recovery, and return to an optimal level of work and functioning.
  • Demonstrate knowledge, skills, and competency in case management standards of practice.
  • Use advanced knowledge of injury types, medications, comorbidities, treatment options, and job duties to advise on treatment plans.
  • Interview disabled persons to assess recovery, including occupational or non-occupational injuries.
  • Collaborate with physicians/providers and utilize available resources to create and implement tailored treatment plans.
  • Work with employers and physicians to modify job duties to facilitate early return to work.
  • Evaluate and modify case goals based on patient progress and treatment effectiveness.
  • Manage workload independently, prioritize cases, and decide on effective case management strategies.
  • Attend appointments when appropriate, prepare status updates, and perform other duties as assigned.
Qualifications
  • Associates Degree or Bachelor’s Degree in Nursing or related field.
  • 2+ years clinical practice preferred; workers’ compensation experience is a plus.
  • Ability to advocate effectively with physicians, employers, and customers. Independent work skills. Basic computer skills (Excel, Word, Outlook). Strong written communication skills.
  • Active Registered Nurse (RN) license in good standing.
  • Reliable transportation and ability to travel to in-person appointments in assigned geography.
  • Reliable internet connection.
Benefits

Our total compensation package supports health, wealth, and well-being, including Medical, Dental, Vision, HSA/FSA, Life and AD&D Insurance, 401(k), Tuition Reimbursement, and wellness resources. The annual base pay ranges from $80,000 - $90,000, with eligibility for a productivity-based bonus. Total compensation depends on skills, experience, education, and performance.
