$89,158.00/yr - $161,015.00/yr

Summary

The Remote Patient Monitoring - Home Telehealth Case Manager is responsible for the nursing process when providing and supervising direct patient care. The RN coordinates care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. The role involves care, counseling, and health promotion based on the nursing process.

Duties
  • Apply care and case management principles using health informatics, disease management, and HT technologies to facilitate access to care and improve Veterans' health.
  • Improve clinical outcomes and access to care, reducing complications, hospitalizations, and emergency visits for high-risk patients.
  • Conduct initial and ongoing assessments to establish a comprehensive care plan.
  • Triages and assesses data from HT Patients, including vital signs and symptoms.
  • Identify and intervene in potential exacerbations to facilitate timely care.
  • Communicate with community-based providers for co-managed care.
  • Provide interdisciplinary consultation and interventions with various care programs and services.
  • Identify patients' self-management skills and knowledge gaps.
  • Provide health coaching, education, and motivational counseling.
  • Coordinate with interdisciplinary teams regarding patient progress and data patterns.
  • Offer feedback for colleagues and foster a supportive environment for professional development.
  • Participate in evidence-based practice activities and share educational findings.
  • Perform other duties as assigned.

Requirements

Qualifications

Must meet basic education and licensure requirements, including:

  • Proficiency in English.
  • Graduate of an accredited nursing program.
  • Current, active, and unrestricted RN license in the U.S.

Preferred experience includes 2-3 years of RN experience and case management background.
