Ascend Hospice & Palliative Care VA is hiring an RN Case Manager to support on-call coverage, case management, and in-home visits in the Richmond/Mechanicsville, VA area. The role requires an Associate's Degree in Nursing and an active State RN license, with 2 years hospice nursing experience preferred.

Qualifications

• Must be willing to travel.
• Strong verbal and written communication skills.

Responsibilities

• Complete initial patient and family assessments to determine hospice care needs.
• Regularly re-evaluate patients' nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with healthcare team, patient and family, to coordinate care.
• Counsels patient and family on end-of-life needs.
• Initiate preventative and palliative nursing procedures.
• Communicate with physician about patient needs and changes in condition.
• Supervise Hospice Aides.

Now Hiring – RN Case Manager (Richmond/Mechanicsville, VA)

Hours: Monday - Friday 8:30am - 5:00pm

The RN Case Manager will be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

• Completing an initial assessment of patient and family to determine hospice care needs
• Regularly re-evaluates the patients nursing needs
• Use assessment data to determine care plan needs
• Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide care that is continuous and well-coordinated
• Counsels the patient and family in meeting end of life needs
• Initiates appropriate preventative and palliative nursing procedures
• Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
• Supervises the Hospice Aides
