Job summary

Ascend Hospice & Palliative Care VA in Richmond, VA seeks an RN Case Manager to coordinate end-of-life care for patients and families. A $5,000 sign-on bonus is offered, with competitive benefits and opportunities to grow within the Ascend family.

The role requires an active RN license, 2 years hospice nursing experience if possible, travel as needed, and strong communication with the care team to ensure continuous, compassionate care.

Qualifications

• State Certified RN license required.
• 2 years professional nursing experience; hospice experience preferred.
• Valid driver's license and reliable transportation.
• Willingness to travel as needed.
• Strong verbal and written communication skills.

Responsibilities

• Complete an initial patient and family assessment to determine hospice care needs.
• Regularly re-evaluate patients’ nursing needs.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient, and family to coordinate care.
• Counsels the patient and family in meeting end-of-life needs.
• Initiate appropriate preventative and palliative nursing procedures.
• Communicate with the physician regarding patient needs and changes.
• Supervise the Hospice Aides.

TheRN Case Managerwill be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

• Completing an initial assessment of patient and family to determine hospice care needs
• Regularly re-evaluates the patients nursing needs
• Use assessment data to determine care plan needs
• Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated
• Counsels the patient and family in meeting end of life needs
• Initiates appropriate preventative and palliative nursing procedures
• Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
• Supervises the Hospice Aides
