Job summary

At Home Care Hospice is seeking a passionate Hospice RN Case Manager to join our team in Williamsburg, VA. You will provide expert, patient-centered care, creating individualized care plans and collaborating with an interdisciplinary team in a supportive environment.

Responsibilities include assessing patient needs, administering medications, educating families, and participating in on-call rotation. Travel across the territory is required, with a schedule Monday–Friday, 8 AM–5 PM, and a $5,000

Qualifications

• Graduate of an accredited nursing program with current state licensure.
• Minimum 1 year nursing experience; 3+ years preferred.
• Hospice experience and good documentation practices encouraged.

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligned with patient wishes and goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management for comfort.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team to coordinate care.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation
• One year nursing experience required , 3+ years preferred
• Experience in hospice or a similar setting preferred
• Knowledge of the hospice philosophy of care
• Commitment to clinical and documentation excellence
• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order
