Job summary

Ascend Hospice & Palliative Care VA in Short Pump, VA is hiring an RN Case Manager to support on-call coverage, case management, and home visits. You will assess patients and families, develop care plans, and coordinate with the healthcare team to ensure continuous and well-coordinated care.

You must hold a State Certified RN license and have at least 2 years of nursing experience, with hospice experience preferred; a valid driver's license and willingness to travel are required.

Qualifications

• State-certified RN license required.
• Associate's Degree in Nursing required.
• 2+ years of nursing experience with hospice preferred.
• Must be willing to travel.

Responsibilities

• Complete initial patient and family assessments to determine hospice care needs.
• Regularly re-evaluate patient nursing needs and update care plans.
• Collaborate with the healthcare team to coordinate continuous and well‑organized care.
• Counsel patients and families on end-of-life needs and preferences.
• Initiate appropriate preventative and palliative nursing procedures.
• Communicate changes in patient condition with the physician.
• Supervise Hospice Aides.

Now Hiring –RN Case Manager –Short Pump, VA

TheRN Case Managerwill be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

• Completing an initial assessment of patient and family to determine hospice care needs

• Regularly re-evaluates the patients nursing needs

• Use assessment data to determine care plan needs

• Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated

• Counsels the patient and family in meeting end of life needs

• Initiates appropriate preventative and palliative nursing procedures

• Communicates with the physician regarding patient's needs and reports any changes in the patient's condition

• Supervises the Hospice Aides
