Job summary

Ascend Hospice & Palliative Care VA is hiring an RN Case Manager in Midlothian, VA. The role supports On Call/Case Management/Visits to ensure continuous, well-coordinated care for patients and families. A $5,000 sign-on bonus is offered.

The ideal candidate holds an RN license, has 2 years of nursing experience (hospice preferred), and is willing to travel. Competitive benefits and opportunities to grow are provided.

Qualifications

• Must have a State Certified RN license.
• 2 years' nursing experience; hospice experience preferred.
• Must have a valid Driver's license and reliable transportation.
• Must be willing to travel.
• Strong verbal and written communication skills.

Responsibilities

• Perform initial patient and family assessments to determine hospice care needs.
• Regularly re-evaluate patients' nursing needs and adjust care plans.
• Use assessment data to determine care plan needs.
• Collaborate with the healthcare team, patient, and family for coordinated care.
• Counsel patient and family on end-of-life needs.
• Initiate appropriate preventative and palliative nursing procedures.
• Communicate with physicians regarding patient needs and changes in condition.
• Supervise Hospice Aides.

TheRN Case Managerwill be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

• Completing an initial assessment of patient and family to determine hospice care needs

• Regularly re-evaluates the patients nursing needs

• Use assessment data to determine care plan needs

• Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated

• Counsels the patient and family in meeting end of life needs

• Initiates appropriate preventative and palliative nursing procedures

• Communicates with the physician regarding patient's needs and reports any changes in the patient's condition

• Supervises the Hospice Aides
