Key Responsibilities

Careline Hospice Registered Nurses are trained professionals who administer skilled nursing care, case management, and support to terminally ill patients and their families in home or facility settings. They access, plan, coordinate, implement, and evaluate patient care plans. In collaboration with the Hospice Medical Director, they lead an interdisciplinary team that may include a Hospice Aide, Medical Social Worker, Chaplain, LPN, Music Therapist, Massage Therapist, and Volunteers to provide comprehensive care.

In This Role You Will Be Responsible For
  • Completing initial and ongoing evaluations of patients’ nursing needs during visits.
  • Managing caseloads by assessing, planning, coordinating, and evaluating patient care plans.
  • Assessing home safety risks, environment suitability, support systems, and community resources.
  • Providing support at the time of death and performing bereavement assessments if requested.
  • Communicating effectively with physicians, patients, families, facilities, visitors, and staff.
Minimum Qualifications
  • Graduate of an accredited nursing program (Diploma, Associate, or Baccalaureate).
  • Current State Registered Nurse license.
  • At least one year of clinical nursing experience.
  • Proficient nursing skills per accepted standards.
  • Ability to assess and respond to patient and family needs in various settings.
  • Reliable transportation and valid auto insurance.
  • Physical ability to lift at least 35 lbs and perform necessary movements.
Preferred Qualifications
  • BSN degree.
  • Experience in hospice or home health care.
  • Familiarity with Home Care Home Base (HCHB).
Location & Hours
  • Office: 24255 W 13 Mile Rd STE 210, Bingham Farms, MI 48025
  • Main Service Area: Wayne County
  • Hours: 8:00 am – 5:00 pm, Monday through Friday, with weekend and on-call rotation.
