We are seeking a full-time, 0.8 - 1.0 FTE (32-40 hours/week), benefited Field Nurse Case Manager to cover the greater Detroit, MI area.

This is a home-based role with travel up to 2 hours one-way to various locations such as doctors’ offices and hospitals. The individual will be responsible for medical case management of work-related injuries, including assessment, planning, coordination, implementation, and evaluation of injured/disabled individuals. Autonomy and relationship-building with injured workers, providers, insurance companies, employers, and attorneys are essential.

Qualifications:

• Current, unencumbered RN license in MI.
• Preferred certifications: CCM, CRRN, CDMS, CRC (required within two years of hire).
• Bilingual Spanish is a plus.
• Valid driver’s license with a good driving record.
