Responsibilities include assuming full nursing responsibility for the delivery of the Plan of Care for each hospice patient, evaluating performance, and making necessary improvements to increase productivity and quality of care.

• Making initial nursing evaluations within 48 hours of referral to determine eligibility for hospice services.
• Identifying and reassessing the patient's physical, psychosocial, and environmental needs at least every 15 days.
• Initiating and coordinating the plan of care.
• Documenting problems, goals, interventions, and patient/family responses.
• Collaborating with the patient/family, attending physician, and other team members daily.
• Instructing and supervising the patient/family in self-care techniques when appropriate.
• Maintaining accurate clinical notes regarding the patient’s condition.

Experience Requirements:

• At least one year of clinical experience.

License Requirements:

• Current RN licensure in the state of practice.
• Current CPR Certification.
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.

Additional State Specific Requirements:

• LA: Minimum two years full-time RN experience or two years of hospice care as a licensed practical nurse as a substitute.
