Job summary

Spirit Hospice of Georgia is seeking a dedicated RN Case Manager to serve as the clinical anchor for patients and families. You will coordinate end-of-life care with a multidisciplinary team, ensuring compassionate, informed, and personalized support throughout the care journey.

We value teamwork and empathy, with a focus on patient-centric care. A Georgia RN license and CPR/First Aid certification are required, along with reliable transportation to patients' residences.

Qualifications

• Reliable and legal transportation to patient residences.
• Hospice or home care experience is a plus, but kindness matters most.

Responsibilities

Position Requirements

• Current RN license in the State of Georgia
• Current CPR & First Aid Certification
  
• Reliable and legal transportation to and from patient's place of residence
