Job summary

H. T. PROF Group is seeking a full-time RN Case Manager in the Kansas City, MO area.

You will coordinate and oversee hospice patient care, develop individualized plans, perform assessments, and work with the interdisciplinary team to support patients and families. This role requires MO RN license, valid driver's license, 2 years RN clinical experience, and strong clinical decision making. Hospice experience preferred; benefits include medical/dental/vision, life insurance, HSA/FSA, mileage

Qualifications

• Current Registered Nurse license valid for Missouri
• Current driver's license
• Two years of RN experience in a clinical setting
• One year of hospice experience preferred
• Reliable transportation and auto liability insurance
• Ability to make sound clinical decisions
• Strong written and verbal skills

Responsibilities

• Coordinate and oversee all aspects of assigned patients' hospice care
• Develop and implement individualized care plans with the interdisciplinary team
• Perform head-to-toe assessments, monitor symptoms, and manage medications
• Educate patients and families regarding conditions and needs
• Communicate patient progress with leadership, physicians, and care team
• Complete required documentation accurately and on time

Outstanding hospice provider seeks a full-time RN Case Manager. This nurse will coordinate and oversee patient care, develop and implement individualized plans of care, provide clinical assessment and symptom management, and work closely with the interdisciplinary team to support patients and families.

Required Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience preferred
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills

Key Responsibilities

• Coordinate and oversee all aspects of assigned patients' hospice care
• Develop and implement individualized plans of care with the interdisciplinary team

• Educate patients and families regarding condition, needs, and care
