Job summary

The Keystone Distinction Group in Kansas City Metro, MO (east) is seeking a full-time Hospice Admissions RN to coordinate new patient admissions, perform comprehensive assessments, educate patients and families, and partner with the interdisciplinary care team to ensure a smooth transition into hospice services.

You will be responsible for collaborating with care teams, educating families, and ensuring accurate documentation to support high-quality end-of-life care.

Qualifications

• Current Registered Nurse license valid for practice in Missouri.
• Current driver’s license.
• Current CPR certification.
• Two years of Registered Nurse experience in a clinical care setting.
• One year of full-time hospice experience.

Responsibilities

• Coordinate and oversee all aspects of new patient admissions.
• Collaborate with patient care teams to support seamless communication and continuity of care.
• Educate patients and families regarding condition, needs, and care.
• Perform head-to-toe physical assessments.
• Complete all required documentation accurately and on time.

Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.

Required Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Current CPR certification
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills

Key Responsibilities
