Job summary

The Keystone Distinction Group is hiring a Hospice RN Case Manager in the Kansas City metro (east), MO, for a full-time role. You will coordinate and oversee hospice patient care, develop individualized plans, and support families with symptom management.

The position requires Missouri RN license, CPR, a valid driver's license, and at least two years of RN experience in clinical care; hospice experience preferred. Benefits include comprehensive coverage and PTO.

Qualifications

• Current Registered Nurse license valid for Missouri.
• Current driver's license.
• Current CPR certification.
• Two years of RN experience in a clinical setting.
• One year of hospice experience preferred.
• Reliable transportation and auto liability insurance.
• Ability to work independently and make sound clinical decisions.
• Strong written, verbal, organizational and interpersonal skills.

Responsibilities

• Coordinate and oversee all aspects of assigned patients' hospice care.
• Develop and implement individualized plans of care with the interdisciplinary team.
• Perform head-to-toe assessments, monitor symptoms, and manage medications.
• Educate patients and families regarding condition, needs, and care.
• Communicate patient progress with clinical leadership, physicians, and the care team.
• Complete all required documentation accurately and on time.

Hospice RN Case Manager

Outstanding hospice provider seeks a full-time RN Case Manager. This nurse will coordinate and oversee patient care, develop and implement individualized plans of care, provide clinical assessment and symptom management, and work closely with the interdisciplinary team to support patients and families.

Required Qualifications
