Job summary

H. T. PROF Group in Kansas City, MO is seeking a full-time Admissions RN to coordinate new patient admissions, perform comprehensive assessments, educate patients and families, and partner with the interdisciplinary care team to ensure a smooth transition into hospice services.

The role requires Missouri RN license, valid driver's license, two years acute care RN experience and one year hospice experience, along with reliable transportation and strong communication skills.

Outstanding hospice provider seeks a full-time Admissions RN. This nurse will coordinate new patient admissions, complete comprehensive assessments, educate patients and families, and partner closely with the interdisciplinary care team to ensure a smooth transition into hospice services.

Required Qualifications

• Current Registered Nurse license valid for practice in Missouri
• Current driver's license
• Two years of Registered Nurse experience in a clinical care setting
• One year of full-time hospice experience
• Reliable transportation and valid automobile liability insurance
• Ability to work independently and make sound clinical decisions
• Strong written, verbal, organizational, and interpersonal skills
