Job summary

Interim HealthCare Hospice in the Kansas City metro area is seeking a compassionate Hospice Registered Nurse (RN) to join our growing team. You will deliver patient-centered end-of-life care at home, coordinating care plans, educating families, and partnering with physicians and the interdisciplinary team.

The ideal candidate has an active RN license in KS/MO (or ability to obtain within 30 days) and at least 1 year of RN experience, with hospice/home health experience preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Current, active RN license in Kansas and Missouri (or ability to obtain within 30 days).
• License must be in good standing.
• At least 1 year of RN experience.
• Hospice, home health, palliative care, oncology, or medical-surgical experience preferred.

Responsibilities

• Manage and coordinate hospice patient care.
• Complete comprehensive patient assessments.
• Develop and update individualized plans of care.
• Educate and support patients and their families.
• Collaborate with physicians and the hospice team.
• Maintain accurate and timely clinical documentation.
• Participate in on-call rotation as assigned.

• Valid driver's license, reliable transportation, and current auto insurance
• Strong assessment, communication, and organization skills
