Job summary

Phoenix Home Care and Hospice in Missouri is seeking a Registered Nurse Case Manager to provide compassionate nursing care to clients and families facing a terminal diagnosis. You will collaborate with the interdisciplinary team to develop and implement a patient/family centered plan of care, including goals of care discussions and guidance on healthcare decisions to improve quality of life.

Qualifications

• Missouri RN license or Compact RN license required.
• Valid driver's license and auto liability insurance.
• Missouri Compact license accepted where applicable.

Responsibilities

• Educate patients and families about the process, plan of care, and healthcare decisions guidance.
• Consult with attending physician and Hospice Medical Director regarding changes to patient condition or plan of care.
• Coordinate hospice care with the team and families to ensure needs are met.
• Conduct initial and ongoing comprehensive assessments.
• Perform hospice care evaluations and informational visits.

Phoenix Hospice RN Case Managers provide compassionate nursing care to clients and their families dealing with a terminal diagnosis. Will work with the interdisciplinary team to develop and implement a plan of care that is patient/family centered, with goals of care discussions and guidance with healthcare decisions to give quality of life.

Responsibilities

• Provide education to patients/family members regarding the process, plan of care, and offers guidance with healthcare decisions
• Consult with the attending physician and Hospice Medical Director concerning changes in patients condition requiring order changes and/or changes to plan of care
• Coordinate hospice care by working with other team members as well as with the patients/family members to ensure needs are met
• Conducts initial as well as on-going comprehensive assessments
• Perform evaluations for hospice care along with informational visits

Requirements
