JourneyCare Hospice is seeking an RN Case Manager to join our growing care team in Woodstock, IL. The role supports patients and families through all phases of care with regular visits and coordination of the interdisciplinary team.

The position requires an accredited RN program graduate, current RN license in Illinois, CPR and First Aid certifications, and a driving license. We offer competitive benefits and growth opportunities within a supportive culture.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR, First Aid Certification.
• Knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process.
• Obtain data and assess information from patient physical, psychological, social, and spiritual factors that may impact needs and initiate intervention by the interdisciplinary team.
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for coordinating optimal care.
• Provide regular visits to re-assess the patient’s needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely.
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines.
