Job summary

Capital City Hospice in Columbus, OH is seeking an RN Float for hospice care. You will serve as the primary RN for patient and family caseload, lead admissions, and develop individualized plans of care in collaboration with the interdisciplinary team.

Overview includes assessing eligibility, coordinating interventions, and maintaining detailed documentation. A compassionate communicator with strong clinical judgment is essential to support patients and families through life’s journey.

Qualifications

• Graduate of an accredited RN program.
• Current RN license in the state of practice.
• Current CPR and First Aid certification.
• Valid driver’s license and reliable, insured vehicle.
• Strong clinical judgment and ability to make independent decisions.

Responsibilities

• Serve as the primary RN for your patient and family caseload.
• Complete assessments and determine hospice eligibility.
• Lead the admission process and develop individualized plans of care.
• Evaluate needs and coordinate interventions with the interdisciplinary team.
• Communicate with attending physicians, team members, and partner agencies to ensure seamless care.
• Conduct regular patient visits to reassess needs and update care plans.
• Perform clinical pronouncements of death and provide compassionate support to families.
• Maintain accurate, timely documentation, care plans, and progress notes.
• Supervise and document care provided by Home Health Aides per guidelines.

What You’ll Do

• Communicate with attending physicians, team members, and partner agencies to ensure seamless care
• Conduct regular patient visits to reassess needs and update care plans
• Perform clinical pronouncements of death and provide compassionate support to families
• Maintain accurate, timely documentation, care plans, and progress notes
• Supervise and document care provided by Home Health Aides per federal, state, and company guidelines

What You Bring

• Graduate of an accredited registered nursing program
• Current RN license in the state of practice
• Current CPR and First Aid certification
• Knowledge of quality assessment and assurance practices
• Strong clinical judgment and ability to make independent decisions
• Compassionate communication skills and the ability to support patients and families
• Valid driver’s license and reliable, insured vehicle
