Job summary

Capital City Hospice is seeking a Registered Nurse (RN) for Hospice On Call / Triage in Columbus, OH and surrounding areas. You will manage patient and family caseloads, assess needs, determine eligibility, and coordinate care with the interdisciplinary team during on-call shifts, including after-hours coverage.

The role requires an accredited RN program graduate, active RN licensure in Ohio, CPR certification, a valid driver's license, and strong interpersonal skills to support patients,

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Maintains a current CPR certification.
• Must possess the ability to deal tactfully with patients, family members, visitors, agency personnel, and general public.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assist with patient and family caseload management across assessment, eligibility, admission processing, planning, implementation and evaluation phases of the nursing process.
• Obtain data from patient’s physical, psychological, social and spiritual factors to plan care with the interdisciplinarity team.
• Initiate communication with attending physician and other agencies to coordinate optimal care.
• Provide visits to reassess patient needs and update the care plan.
• Perform nursing procedures as directed by physician.
• Provide support during clinical death pronouncement and support to family.
• Maintain accurate patient records and care plans in a timely manner.

What You’ll Do:

• Assist with the responsibility for patient and family caseload, including the assessment, determine eligibility, admission processing, planning, implementation and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social and spiritual factors that may impact patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team and other agencies for the purpose of coordinating optimal care
• Provide visits to re-assess the patient's needs and update the plan of care
• Perform therapeutic, preventative and rehabilitative nursing procedures as directed by the attending physician
• Perform clinical pronouncement of death and provide support to family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely

Qualifications:

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Maintains a current CPR certification.
