Job summary

Trustbridge Hospice, part of Empath Health, is seeking an experienced RN Hospice Case Manager to coordinate care for patients and families across Southern Palm Beach County.

You will advocate for patients, collaborate with the interdisciplinary team, and develop individualized plans of care, ensuring comfort and quality of life. This role requires Florida RN licensure and strong teaching skills, with a schedule of 8:30am–5pm and alternating weekends.

Qualifications

• Current Florida RN license required.
• Experience in hospice, home health, or geriatrics preferred.
• Strong communication and teaching skills.
• Reliable transportation and valid driver’s license.
• Proficient in basic computer skills.

Responsibilities

• Coordinate care with the interdisciplinary team to develop plans of care.
• Advocate for patients and families throughout the hospice journey.
• Monitor patient conditions and provide clinical interventions.
• Educate patients and families to improve comfort and quality of life.
