Job summary

LilyCare Hospice in Albuquerque, NM is hiring a full-time Hospice Registered Nurse Case Manager to join our clinical team and help shape hospice care delivery from the ground up. You will manage a caseload in private homes and facilities, complete assessments, develop plans of care, provide pain and symptom management, and coordinate with the attending physician and Hospice Medical Director.

The role requires EMR documentation and participation in on-call rotations, with a Monday–Friday schedule

Qualifications

• Active RN license in New Hampshire or a compact state.
• One year of RN experience; hospice, home health, or case management preferred.
• Current CPR certification.
• Valid driver’s license.

Responsibilities

• Serve as the Hospice Registered Nurse Case Manager for a caseload of hospice patients in private homes and facilities - complete assessments, individualized plans of care, and routine visits
• Provide skilled pain and symptom management in coordination with the attending physician and Hospice Medical Director
• Participate in IDG meetings and coordinate care across the interdisciplinary team, including hospice aides, social work, and chaplaincy
• Educate and support patients, families, and caregivers, and supervise hospice aide care per the plan of care
• Complete timely, accurate EMR documentation and participate in the RN Case Manager on-call rotation

Compensation and Schedule

The Hospice Registered Nurse Case Manager role pays $90,000 - $105,000/year based on experience. Full-time, Monday-Friday day schedule with a rotating on-call assignment. Territory is based out of Albuquerque, NM.
