Atlantic Health in Morristown, NJ, seeks an RN for hospice case management and end-of-life care coordination. You will evaluate plans of care, reconcile medications, and engage in interdisciplinary teamwork with physicians, social workers, and other clinicians.

The role emphasizes teaching, mentorship, and orientation for new staff while supporting growth initiatives within the agency. A strong foundation in pharmacology and patient-centered planning is essential.

Formación

• Graduate of an accredited Nursing Program.
• Current state licensure as a Registered Nurse.
• BLS certification.
• Prefers BSN; ASN must complete BSN within five years; Hospice nursing experience may be preferred.

Responsabilidades

• Assumes responsibility for case management.
• Prioritizes schedule to accommodate patient's needs.
• Evaluates the plan of care and revises as needed.
• Demonstrates knowledge of pharmacology and safe medication management and end-of-life symptom management.
• Routinely reconciles medications.
• Maintain current medication orders.
• Shows evidence of interdisciplinary communication.
• Participates in PI meetings and activities/committees as requested.
• Assists with growing agency business.
• Provides orientation to new employees.
• Accepts other assignments as requested.
• Cross trains to learn new skills and demonstrates a willingness to teach others.
