Job summary

Gentiva in Myrtle Beach, SC seeks a dedicated Hospice Patient Care Manager to lead clinical operations and ensure compliant, high-quality hospice services. You will oversee patient care, coordinate visits, and mentor staff while maintaining regulatory standards.

Reporting to the Executive Director, you will drive performance improvements, manage IDG meetings, and support care delivery at our location. RN license and hospice experience are required with strong leadership capability.

Qualifications

• Current RN license in the state of employment.
• Graduate of an accredited nursing program.
• Minimum of 3 years of direct patient care experience, including 2 years in hospice or home-based care.

Responsibilities

• Oversee and direct patient care services to ensure high-quality hospice care.
• Manage visit scheduling and prioritization of reassigned, missed, declined, or rescheduled visits.
• Triage incoming referrals and medical concerns in a timely, compassionate manner.
• Lead or facilitate Interdisciplinary Group (IDG) meetings and document minutes.
• Mentor, train, and provide clinical support to associates for development and compliance.
• Ensure care delivery meets federal, state, and local regulatory standards and company policies.
• Support performance improvement initiatives and operational efficiencies.
