Job summary
Gentiva is seeking an experienced Registered Nurse Manager to lead hospice patient care in North Charleston, South Carolina. The role oversees clinical operations, coordinates care plans, mentors staff, and ensures regulatory compliance across the care continuum.
The ideal candidate brings RN licensure in SC, hospice or home-based care experience, and strong leadership in documentation and performance improvement. This is a full-time on-site role with comprehensive benefits.
Qualifications
- Current RN license in the state of employment.
- Graduate of an accredited nursing program.
- Minimum of 3 years of direct patient care, including hospice or home-based care.
- Knowledge of hospice principles and regulatory compliance (Medicare/ Medicaid/ JCAHO/ ACHC).
Responsibilities
- Oversee and direct patient care to ensure high-quality hospice services.
- Manage visit scheduling and prioritize based on clinical judgment.
- Triage referrals and medical concerns promptly and professionally.
- Lead IDG meetings, present updates, coordinate care plans, document minutes.
- Mentor and train associates to support development and compliance.
- Ensure care delivery meets federal, state, and local standards and policies.
- Support quality improvement and operational efficiency at the location.
