Job summary

VitalCaring is seeking a Clinical Care Manager (RN) – Hospice in Round Rock, TX to guide patients and families through end-of-life care with compassion and clinical excellence.

You will lead the Plan of Care, coordinate across physicians, social workers, chaplains, and care teams, and ensure comfort, dignity, and timely, high-quality hospice services. This full-time role offers growth, supportive leadership, and a mission-driven workplace.

Qualifications

• Active RN license in state of practice.
• Graduate of an accredited nursing program (BSN preferred).
• Hospice or Home Health experience required.
• 2+ years of leadership or care coordination experience.
• Strong communication skills for patients, families, and care teams.

Responsibilities

• Coordinate and oversee hospice patient care across the interdisciplinary team.
• Serve as the central point of communication between physicians, caregivers, patients, and internal teams.
• Manage physician orders and ensure timely entry and alignment with the plan of care.
• Partner with Intake to support timely admissions and follow up on pending referrals.
• Continuously assess patient needs and ensure appropriate reassessment with any change in condition.
• Lead development, implementation, and updates of individualized hospice plans of care.
• Support patients and families through education, symptom management, and end-of-life care planning.
• Review clinical reports and take appropriate action.
• Participate in IDG meetings and care planning.
• Perform in-home visits as needed to support care delivery and team alignment.
• Ensure compliance with hospice regulations, documentation standards, and quality expectations.
• Identify trends or gaps in care and escalate to Branch leadership with recommendations.
