Qualifications

• Active RN license in state of practice is required.
• Graduate of an accredited nursing program, BSN preferred.
• Previous hospice or home health experience with understanding of end-of-life care is essential.
• 2+ years of leadership or care coordination experience preferred.
• Strong clinical judgment, particularly in symptom management.

Responsibilities

• Coordinate and oversee hospice patient care across the interdisciplinary team.
• Serve as the central communication point between caregivers and internal teams.
• Lead the development of individualized hospice plans of care.
• Support patients and families through education and end-of-life care planning.
• Ensure compliance with hospice regulations and quality expectations.
