Job summary

Interim HealthCare Hospice & Palliative Care in Indiana is seeking a Hospice Patient Care Coordinator (RN) to oversee care and services delivered by staff for our hospice patients. The role focuses on ensuring quality, compliance with physician orders, and patient-centered outcomes.

You will collaborate with clinical leadership, supervise staff, provide ongoing education on hospice regulations and standards, and participate in admissions.

Qualifications

• RN licensed in Indiana with active, in-good-standing license.
• Minimum 2 years as a Hospice RN.
• At least 1 year of supervisory experience preferred.
• Valid driver's license and auto liability insurance.

Responsibilities

• Collaborate with the Director of Clinical Services to develop and implement processes to ensure patient outcomes per physician orders.
• Provide ongoing education to hospice employees on regulations, standards, policies, QAPI requirements.
• Supervise, evaluate, motivate, and develop professional and paraprofessional staff.
• Oversee clinical decision making to manage assigned patient population.
• Serve as an expert for hospice admissions and perform admissions as assigned.
