You will conduct evaluations, craft care plans, manage symptoms such as pain and wounds, advocate for patients, and collaborate with a multidisciplinary team to ensure seamless continuity of care.

Qualifications

• Active RN license in good standing.
• Experience in home health or facility-based care.
• Strong nursing judgment and critical thinking.
• Excellent communication with patients and families.

Responsibilities

• Conduct comprehensive evaluations and ongoing assessments of patient needs.
• Develop, implement, and adjust care plans for end-of-life care.
• Educate patients and families on Advance Directives and care options.
• Collaborate with healthcare professionals to ensure continuity of care.
• Provide compassionate, culturally sensitive support throughout the care journey.
• Coordinate scheduling and logistics for home health visits.
