Job summary

Center4Hospice in Indiana is seeking an experienced RN with hospice and management experience to join our team as Patient Care Coordinator. This full-time exempt role supports nursing leadership and on-call rotation to ensure high-quality patient care.

You will supervise staff, coordinate patient care, ensure compliance with nursing standards and regulations, and participate in quality improvement initiatives within the agency.

Qualifications

• Maintains current R.N. licensure required in Indiana.
• Previous supervisory experience required.
• Previous nursing experience in home health/hospice a plus.

Responsibilities

• Manages resources, facilitates concepts and approaches, reviews projects' progress and results; makes management decisions.
• Ensures compliance with nursing care standards established by commissions, and federal and state regulatory agencies, and adherence to Agency objectives and policies pertaining to nursing administration.
• Participates in the selection, hiring and termination process of staff
• Determines workload and delegates assignments, training, monitoring and evaluating performance, and initiating corrective or disciplinary actions in accordance with Agency policy and in a timely manner.
• Responsible for meeting the requirements for contracted staff.
• Responsible for the scheduling and coordination of patient care staff.
• Acts as a resource to provide information or determine the most effective way of meeting the needs of staff, patients, or customers.
• Reviews and assures accuracy of invoices related to areas of responsibility, in a timely manner.
• Responsible for supplies and PAR levels.
• Participates in nursing leadership on call rotation schedule related to clinical issues.
